Before you come to your first visit, please print out this new patient form, fill it out, and bring it with you to your first visit with us.  You may also mail it to us in the enclosed postage-paid envelope, whichever is more convenient for you.  Since we are primarily a paperless dental office (saving the environment by not supporting the cutting down of additional trees to make paper), we would greatly appreciate it if you could go to our website and fill out your information online and send it back to us via the internet.  Your information will be secure and confidential.
